2022/2023 Class Registration

Please only fill out this form if you are ready to register for in studio classes. If you are seeking additional information or to set up a trial class please email us at info@creativeforcedance.com.

When filling out this form be sure to include the Day, Time and the full Name of the Class that you are registering your dancer in, as well as the date they are planning to start taking class. Once you have submitted this form you are responsible for the registration fee and first month of tuition.  Initial payment can be made on the Payment tab of the website, tuition information can be found under the Classes – Fees tab on our website. Payment must be made immediately and prior to your dancer attending their first class. Once you’ve submitted your registration form you should receive an automated reply from our system, please check your junk/spam folder.
